Sunflower Developmental Pediatrics: New Patient Evaluations

 

We recognize that no one knows your child better than you do and feel that your responses will help us learn more about him or her.  The process of evaluating development and behavior in any child is fairly complex, and the more information we have on hand from a variety of sources, the more complete the picture will be.  Please try and be as candid and honest as you’re able when completing the documents as it will assist us in delineating a more accurate evaluation, diagnosis, and treatment plan. Only New Patients to Dr. Ellison (never been seen before in another setting), need to complete the New Patient Intake. Every patient needs to complete the Registration Form and Release of Info.

 

Please understand we do not have the capabilities to properly manage patients who have more significant psychiatric problems, such as Bipolar Disorder.  If it is determined by our medical team that your child has a primary psychiatric diagnosis, we will not be able to provide medications or treat them here, as it is not in the best interest of your child.

 

If  your child is school aged, all recent documents from school including report cards/progress reports, Educational Plans, formal screens such as rating scales or testing completed by teachers or therapists should be copied and sent to Sunflower Developmental Pediatrics prior to your first visit along with the completed packet of information.  If your child has had previous evaluations for attention or behavior problems, psychiatric or psychological issues, developmental delays, or learning problems, please include that information as well.  All documents can be received standard mail. Please note, when sending anything via standard mail, only send photo copies; never send originals. Email is not HIPPA compliant. The evaluation process can vary depending on the situation, the needs of your child, and if other portions of an evaluation have already been completed.  The first appointment is scheduled for approximately one hour.  Your child and at least one parent or guardian needs to attend this appointment. If you have other children, it is important to leave them elsewhere, as we want to give our full attention to the child who is being evaluated. During the initial visit, you will discuss your questions and concerns regarding your child's development, school problems, behaviors, and attention concerns. We will also spend time of course with your child.  After that appointment, if we believe that your child needs additional testing to establish a diagnosis, there may also be additional appointments for Psycho-Educational testing or a Developmental Evaluation.  Once the testing process has been completed, Sunflower Developmental Pediatrics’ clinical team will schedule a follow up consult to discuss your child's information with you, review the diagnosis and recommend a multifaceted treatment plan.  We will also present you with a comprehensive written report of our findings.


Although we take several insurance plans at present, this is subject to change. For those with insurances we do not accept, a sliding scale and payment plans are available. You will be responsible for any applicable insurance co-pays, deductibles based on your specific health plan and balances on your account for services not covered by your health plan. Co-pays are expected at the time of each visit. We look forward to meeting you and addressing your concerns.
